I understand and acknowledge that the use of firearms is inherently dangerous, and that there is a risk of serious bodily injury or death associated with their use.*

I voluntarily assume all risks associated with my participation in firearms training and related activities, and agree to release, indemnify, and hold harmless Daniel Gilbert, their instructors, employees, agents, and any other parties associated with the training, from any and all liability, claims, damages, or expenses arising from or in connection with my participation in the training and related activities.*

I certify that I am not under the influence of drugs or alcohol, and that I have no physical or mental condition that would impair my ability to participate in firearms training and related activities safely.*

I acknowledge that I have received and understand all safety instructions and warnings provided by Daniel Gilbert and their instructors, and agree to abide by all safety rules and regulations during the training and related activities.*

I understand that this waiver and release of claims is a legally binding document, and that I am waiving my right to sue Daniel Gilbert, Spade Defensive Solutions, their instructors, employees, agents, and any other parties associated with the training for any injuries or damages that may occur as a result of my participation in the training and related activities.*

I acknowledge that I have read this waiver and release of claims, understand its terms, and voluntarily sign it of my own free will.*
